Creating and Connecting a Custom Form
Accessing the Form Builder
Go to the 'Sites' section, select 'Forms', then choose 'Builder'.

Creating a New Form
Click the "Add Form" button in the top right-hand corner to get started. You can build from scratch or pick a template.

Give your form a clear name.

Adding Questions to the Form
Add your basic questions using the standard fields provided.

For custom questions, use the "Add Object Fields" option.

If the right custom field doesn't exist yet, click '+Add' to create one.

Finalising the Form
Add a submission button (e.g. "Submit") and adjust its width if needed, then click 'Save'.

Connecting the Form to Your Calendar
Go to Calendar Settings in your account and open the calendar you want to link.

In the 'Forms & Confirmation' section, select the form you just created from the dropdown menu.

Click 'Save' to finish.

Testing the Connection
Open your calendar and pick a date to book a test appointment. Either paste the calendar link into a new tab, or book via a funnel connected to your calendar.

The custom form should now appear, ready to collect extra information from your clients.
